• 40–50 patient visits daily; walk-in patients accepted daily.
• Full-time physician and one nurse.
• Many patients receive ongoing chronic-disease care involving multiple medications.

• Ground level in a busy strip mall with ample parking.
• Former pharmacy location with a sink and attached cabinetry available for pharmacy use.
• Pharmacy counters and dispensing fixtures need to be installed.
• Existing clinic build-out includes an office, examination rooms and two washrooms.
• Available for immediate occupancy; pharmacy opening is subject to operator fit-out and required approvals.

FLEXIBLE COST SHARING
