housekeeping, laundry, assisting with dressing and personal grooming. Accompany clients to appointments, or planned social engagements, or
errands. Observe and reporting any changes in the client's physical or mental condition to healthcare professionals and family as required where
appropriate. Provide gentle reminders for clients to take their medications, as directed by a healthcare professionals. Ensure the client's home is safe and comfortable, and assist with mobility and fall prevention.

five years of employment with Canadian Mental Health, Alberta, working with a diverse population with chronic complex needs.
