SOCIAL SERVICER WORKER OR PERSONAL SUPPORT WORKER

Our client, Community Services is a unique and dynamic charitable organization that supports people living with complicated issues such as mental and physical health challenges, use challenges, poverty and homelessness. Serving approximately 13,969 annually and providing 1,788 units of supportive housing last year, Our client is one of Ontario's largest mental health service providers of its kind.
Location: Toronto Shifts: On Call Support – All shift and days available Wage: $18/hr
JOB SUMMARY:
The SSW will attend to the physical, cognitive, mental health and social/recreational needs of eligible residents living at our clients Community Services Program in a way that enhances their dignity and personal empowerment.
• Note: Staff will be required to work in residents' rooms and may therefore be exposed to second-hand smoke.
WHAT YOU WILL DO:
• Independently provide a range of support to residents including monitoring their activities, assess and respond to potential crisis situations.
• Build rapport with each client to enable them to meet the daily requirements of their housing environment and a healthy lifestyle.
• Assist with resident routines, maintain general awareness of the activities or Whereabouts of residents in the house.
• Monitor and report on resident's health and wellbeing; assist with medications.
• Resolve issues related to shared living, mediate conflicts between residents, or between residents and neighbors.
• Carry out crisis intervention activities with serious or unexpected problems.
• Maintain the operation of the house and property by conducting security, fire, health and safety checks.
• Increase residents' awareness of safety issues.
• Administer basic first aid as required and activate the appropriate emergency procedures.
• Identify potential issues and communicate to day staff, or note incidents as required.
• Document all overnight activity, incidents or events in appropriate logs and files.
• Assist residents with physical disabilities or chronic illnesses by providing care in their daily activities.
• Clean and assist clients to clean designated areas of the residence.
• Participate in the administration activities of the program.
• Participate actively as a team member in program delivery activities.
WHAT YOU BRING:
Must Haves
• Degree or Diploma in Human Services or equivalent.
• Understanding of harm reduction, the logic model and psychosocial rehabilitation principles.
• Min six months of experience working with people with Mental Health Challenges and Addictions
• Experience working with the client population.
• Knowledge of issues faced by vulnerable adults with special needs, which will include: addictions, cognitive impairments and low income.
• Knowledge of a housing first approach and understanding of health and support services for transgender and gender fluid identified clients.
• Sensitivity, with an ability to interact with clients in a positive and supportive manner.
• An understanding of the therapeutic relationship with an ability to negotiate with clients on goals and priorities.
• Excellent interpersonal, communication and counselling skills.
• A sound understanding of health and safety standards.
• Good crisis intervention & Problem-solving skills.
• Exceptionally Punctual and on time • Sufficiently computer literate to produce and maintain files, records or statistical information.
• Written skills in keeping records or updating logs.
• Experience in working with limited supervision but as part of a team of professionals.
• Good analytical, time management and organizational skills.
• First Aid and CPR certification with fluency in a second language an asset.
• Understanding of cultural competency and anti-oppression framework
• Reflective of a multi-barriered group, as serviced by the program, a strong asset.
