Job Title: Social Service Worker / Personal Support Worker
Client Location: Downtown Toronto, Scarborough

Rate of Pay: $18.00/hr
Hours of Work: Mon-Fri/ Part Time/Oncall Hours may Vary

Education/Skills/Requirements:

• SSW's – if PSW's are available they will accept them for coverage but don't usually use them for the Ingles site
• Women staff only
• Police Clearance
• UMAB certification is preferred but not mandatory for our staff. Their staff all must have it
• New staff – come in 30-60 minutes before the shift starts and staff on site run through the pertinent information to get them through the shift.

Job Description:
• 24 hour housing support site
• Light house keeping duties
• Support residents with cooking/chores if necessary
• Responsible for safety/well being of physical plant and residents
• Communication of whereabouts of residents
• Abstinent environment – may require making accommodation for residents who have relapsed
• Documentation – we have a system in place for agency staff records

Responsibilities:

• Establish a working relationship with each assigned member.
• Using recovery principles, provide service delivery to members including: informal counselling, health promotion, advocacy, knowledge sharing, research, referrals, regular check ins, practical support.
• Identifying and addressing mental health and addiction issues.
• Documentation: complete health assessments, maintain member files and health records, meeting minutes, and other administrative tasks.
• Assist in the identification and resolution of individual and community issues.
• Work with members to achieve a safe living environment including fire safety, member security, health and safety concerns.
• Carry out the duties of the landlord under the Residential Tenancies Act.
• Carrying out regularly assigned or prescribed duties related to the provision of services and supports.
• Supporting assessment of day to day needs of individuals supported and in the implementation of life plans and monitoring achievement of personal outcomes.
• Providing appropriate personal and physical care including medication administration and crisis response.
• Liaising with friends and family as required to address needs and sharing information on behalf of individuals supported.
• Supporting life skills training and providing emotional support to individuals on a one-on-one or group basis.
• Be familiar with and consistently apply the principles of the client's Mission, Vision, Values and Policies and Procedures.
• Provide solutions to barriers impacting achievement of quality of life. Participate to ensure Personal Outcomes for people are met as defined by each participant. Know and understand what each of the goals is for each resident.
• Implement the actions to support people to achieve the goals identified in his/her Person Centered Plan.
• Provide appropriate supports, tools, etc to ensure the health and safety of the people supported. Ensure compliance to Policies and Procedures associated with Health and Safety as well as Abuse Prevention.
• Consistently follow, where appropriate, Behavioral and Crisis Protocols as documented. Document and report Incidents and Serious Occurrences immediately.
• Ensure the work area is clean and safe; correct or report any safety concerns immediately. Document and report accidents, injuries or near misses immediately.
• Repetitive work bending, twisting, standing for long periods of time
• All other duties as assigned by supervisor


Required Skills, Knowledge, and Ability

• Experience working with homelessness, mental illness, addictions, harm reduction and community resources.
• Demonstrated abilities in accurate and timely record keeping and file management.
• Commitment to anti-oppression and working in a recovery framework.
• Capability to perform physical demands of the job including travel to various housing locations and practical assistance with daily living tasks.
• Knowledge of building maintenance and life safety systems.
• Harm Reduction Experience.
• Ability to work in an organized manner and manage multiple priorities.
• Good verbal, listening and written communication skills.
• Ability to work and communicate with staff and members from diverse backgrounds.
